Download, print, and complete both the “Registration” and “Medical History” forms before your visit. Completing forms at home can significantly shorten the waiting time required to see your surgeon.
Always call the Member Services number on your insurance card to confirm our participation with your insurance plan.

All patients are responsible for obtaining required referral or authorization documents, if needed. Referral or authorization documents from primary care physicians are often needed for HMO insurance plans. It can often take several days to acquire a referral or authorization from your referring physician or insurance plan. We will not be able to evaluate patients if the appropriate referral or authorization documents are not in order.
We can evaluate and treat patients without health insurance. Patients are financially responsible for charges of all services performed. Call or speak to our billing staff for further details.
All patients are financially responsible for co-payments or co-insurance at the time when services are rendered.
We can often evaluate and treat patients who have insurance plans with whom we do not participate. We must verify “Out-of-Network” benefits prior to providing any services.
